I confirm that my contact information and the list of items/accessories are accurate. I will not send loose gaskets, display covers, screws, capacitors, resistors, or other loose parts unless they are listed on this repair order. I authorize evaluation of the listed equipment and understand that diagnostic/bench charges may apply and that electronic repair can require component or non-factory modifications.

Liability Waiver / Best-Effort Repair and Risk Terms

I understand that Lauderdale Speedometer provides diagnosis and repair on a best-effort basis and does not guarantee that an item can be repaired or returned to its original condition. Items may be aged, corroded, previously repaired, modified, brittle, or have hidden defects. I understand that disassembly, testing, cleaning, soldering, programming, component replacement, and other reasonable repair attempts may reveal or result in additional damage or complete failure. To the fullest extent permitted by law, I accept these risks and agree that Lauderdale Speedometer is not responsible for additional damage resulting from reasonable diagnostic or repair attempts, except to the extent caused by gross negligence or willful misconduct.
